We are actively seeking hospice medical social workers who are determined to create supportive and meaningful experiences for patients during their end-of-life journey. In this role, you will conduct psychosocial evaluations and provide ongoing counseling to patients and their families. You will work closely with the care team to deliver emotional support when it is most needed.

Qualifications:
- A heart for serving patients and families and a passion for providing exceptional care
- Education: Master's degree in social work from an accredited school of social work approved by the Council of Social Work Education
- Licensure: Current state license as a social worker
- Experience: Minimum of 2 years of clinical experience, ideally in healthcare or hospice
- Requirements: Reliable transportation is necessary. Ability to sit, stand, bend, and occasionally lift at least 25 lbs effectively.
